Once your appointment is scheduled, you will receive an email confirmation with further instructions via Simple Practice patient portal.
✅ Client Portal & Intake Forms
You will receive an email inviting you to our client portal, where you must complete the required intake forms at least 48 hours before your appointment. Please note: We cannot proceed with your appointment if intake forms are incomplete. If forms remain incomplete without communication, your appointment will be automatically canceled.

- Full payment is required within 48 hours of booking to retain your appointment.
- If payment is not received at least 48 hours before your scheduled appointment, it will be automatically canceled.

🔹 Billing Process:
- All insurances (except Husky and Optum) are billed via Alma and Headway.
- Clients will be required to sign up with Alma and Headway and ensure they are always enrolled in autopay to maintain seamless coverage and appointment scheduling.
- Husky (Medicaid) and Optum are billed directly by our office.

📌 Out-of-Network (OON) Insurance & Reimbursement
If your insurance is not listed above, we are an Out-of-Network (OON) provider. We can provide a Superbill that you can submit to your insurance for potential reimbursement.
Common procedure codes used for reimbursement:
- Psychiatric Evaluation: 90792, or 99204, 99205 with 90833 or 90836
- Follow-Up Appointments: 99214 or 99215 with 90833, or 90836.
- Psychotherapy Sessions: 90835 90837
How to Check Your Out-of-Network Coverage:

Our Ketamine Medication Therapy Package is a structured medication-only program designed to support the prescribing component of your Ketamine-Assisted Psychotherapy (KAP) in collaboration with your chosen KAP therapist. This treatment is intended to enhance your mental health journey by ensuring safe and effective ketamine prescribing, while your therapist provides the therapeutic integration (Not included in our services).
📌 Ketamine Medication Therapy Package Includes:
✔ 4 appointments with our psychiatric ketamine prescriber (Ketamine Psychotherapy component NOT included).
✔ Medication prescribed and sent to your preferred pharmacy.
✔ 2 provider consultations with your Ketamine-Assisted Psychotherapy (KAP) therapist:
- 1 mandatory 15-minute consultation
- 1 optional 15-minute consultation (if the therapist chooses to participate)
📌 Important Notes:
- Therapists charge separately for their consultation and treatment time.
- Pharmacy costs for Ketamine dispensing and compounding are separate and may not be covered by insurance.
- Additional medical clearances (e.g., cardiology, neurology, endocrinology) may be required based on your health history. These may be covered by your insurance.
📌 Deposit & Payment Policy for Ketamine Therapy
✔ A $1,000 deposit is required to schedule the first appointment (Ketamine appointments only).
✔ The remaining $500 balance is due before the second appointment, when medication is prescribed and sent to the pharmacy.
📌 Cancellation & Appointment Compliance Policy
✔ Patients must complete the remaining two appointments within three months after their last integration session with their KAP therapist.
✔ Failure to complete medication appointments within 3 months after the last KAP session will result in forfeiture of the remaining two sessions.
✔ If patients do not utilize the option of two additional follow-ups after the first two appointments, no refund is due, and the service is considered paid in full after the second session when medication is dispensed.
✔ No-call/no-show fee: $150.00
✔ Cancellations require at least 72 business hours' notice to avoid a $150.00 cancellation fee.
📌 Deposit Refund Policy for Ineligible Clients:
If a client is deemed ineligible for Ketamine treatment due to medical, psychiatric, or substance use concerns:
✔ $450.00 will be deducted from the deposit and applied to the cost of the initial evaluation session.
✔ $550.00 will be refunded to the original payment method.
✔ Refunds to an alternate payment method may take up to 30 days for verification and processing.

📌 Standard Appointment Cancellation Policy (Psychotherapy, Medication Management, Coaching, ADHD Services)
✔ 48-hour advance notice is required to cancel or reschedule an appointment.
✔ If you cancel within 48 hours of your scheduled time, a $100 cancellation fee will be charged to the card on file.
✔ Late arrivals exceeding 10 minutes will require rescheduling and will incur a $100 late cancellation fee.
✔ One free cancellation fee waiver is provided per year. (Does not apply to Coaching services).
📌 This policy applies to new and existing clients, including initial consultations and follow-ups.
📌 Ketamine Therapy Cancellation Policy
✔ 72-hour advance notice is required to cancel or reschedule ketamine therapy appointments.
✔ If you cancel within 72 hours of your scheduled time, a $150 cancellation fee will be charged to the card on file.
✔ No-call/no-show appointments for ketamine services will be charged the full session fee of $450.00.

At Mental Serenity Psychiatric Services, the safety and well-being of our clients is our top priority. Controlled medications are prescribed sparingly and only when clinically appropriate. These medications are highly regulated by the Drug Enforcement Agency (DEA) and require strict monitoring.
📌 We follow all state and federal regulations, including reviewing the Prescription Drug Monitoring Program (PDMP) before prescribing controlled substances.
💊 Do We Prescribe Controlled Medications?
✔ Yes, but only under strict guidelines and when medically necessary.
✔ Controlled medications are not the first-line treatment and are only considered when other safer, evidence-based options have been explored.
✔ If prescribed, ongoing monitoring, regular follow-ups, and strict compliance with treatment plans are required.
📌 Benzodiazepine Policy (Xanax, Ativan, Klonopin, Valium, etc.)
Extensive research shows that long-term benzodiazepine use can lead to dependency, cognitive impairment, rebound anxiety, insomnia, and an increased risk of early-onset Alzheimer’s and dementia.
✔ We DO NOT prescribe Benzodiazepines. .
✔ They are only used temporarily while a safer, long-term treatment is being established (such as SSRIs, SNRIs, or therapy) and we reserve this use to our current clients at our discretion.
✔ All patients currently on benzodiazepines will be placed on a gradual tapering plan to ensure safety and minimize withdrawal risks upon evaluation.
✔ We do NOT provide long-term benzodiazepine prescriptions. If long-term benzodiazepine use is your preferred treatment approach, you may need to seek care with another provider.
📌 Additional Controlled Substances Policies
✔ Stimulants (e.g., Adderall, Ritalin, Vyvanse) – Only prescribed after a comprehensive ADHD evaluation and ongoing monitoring.
✔ Sleep Medications (e.g., Ambien, Lunesta) – Limited prescriptions with a focus on behavioral sleep therapy and alternative treatments.
